Application Process & Checklist

KNAT testing for the 2024-2025 school year will start January 2024.

  • First, you must take an initial KNAT placement test (cost is $100). Remediation help is available if needed at no cost post-exam.

  • Second, you should obtain your professional references and bring with you on the day of the KNAT exam.

  • Third, order your official high school transcripts or GED and bring with you on the day of the KNAT exam.

  • Fourth, attend a scheduled interview with the nursing coordinator.

Details about Professional References

  • Two professional references must be submitted on on the schools reference form.

  • At least one should be work related from a direct supervisor. It should be within the last two years and cover at least a year’s acquaintance.

  • The second should be from another coworker or a community member who can speak to your ability to critically think and function in a trustworthy manner.

  • If you have not worked in the past two years, then three community references can be submitted.

  • References cannot be relatives.

All references should be mailed, faxed or emailed per the instructions on the reference sheet.

Details About Official Transcript

  • We suggest you send for your official high school transcript or GED documentation as soon as you schedule your KNAT exam.

  • If you do not bring a sealed HS transcript or GED documentation to the KNAT test, order your Official High School Transcript or GED transcript as soon as possible. All information should be mailed to 352 Gros Blvd., Herkimer, NY 13350 attention: LPN Program. (New York state requires all nursing schools keep an official transcript of a HS diploma or GED documentation in your file.) If you cannot find this documentation, you should contact the nursing coordinator for assistance. The interview process and offer of admission cannot occur without it.

Details About Interview Process with Nursing Coordinator

  • It is the applicant's responsibility to assure that all paperwork has been received by the school.

  • After pretesting has been successfully completed and references and transcripts reviewed, the individual will be contacted by the school for an interview with the nursing coordinator via email.

  • The interview is held at the LPN program school's campus located at the Herkimer BOCES Gateway Center on the second floor of the Arc Herkimer building at 420 E. German St. Herkimer, NY 13350.

  • The interview will center on the program requirements. The applicant’s plans for family responsibilities, transportation and finances will also be discussed. Any concerns students may have involving their ability to be successful in school, on the State Board NCLEX exam or with state licensure should be addressed at this time.

  • Provided admission is offered, it is at the discretion of the LPN coordinator which program will be offered.

Additional note: 

Criminal Records - If you have history of a felony, misdemeanor, have ever been dismissed for patient abuse or are a registered sex offender, you should be aware that you may have difficulty in obtaining clinical site clearance. It may also require a Morals Hearing before being granted licensure by the New York State Office of Professions.

Placement Exam (KNAT)

Herkimer BOCES utilizes the Kaplan Nursing Admission Test (KNAT) as its baseline placement exam. As of January 2022, Herkimer BOCES no longer utilizes the Test of Essential Academic Skills (TEAS exam).

Placement Exam Details

  • Placement testing is held at the LPN program located at the Herkimer BOCES Gateway Center on the second floor of the Arc Herkimer building at 420 E. German St. Herkimer, NY 13350.

  • Cost for the KNAT exam is $100. This must be prepaid and is non-refundable. It is paid online directly to Kaplan. Scheduling cannot be done until the payment is processed. To obtain the payment web information, call our preadmissions office at 315-867-2206 or email sshedd@herkimer-boces.org. After payment is received by Kaplan, they will notify the preadmissions office, and you will be contacted to set up a testing date.


  • It is suggested that you obtain a study guide for the KNAT from Amazon or a Barnes and Nobel book store. A review of the math and ELU sections is particularly helpful.

  • Allow at least three hours to take the test. The test will be 91 questions given on a computer. The test is based on high school level reading, math, science, writing and critical thinking. It is primarily a multiple choice test, and each section is timed.

  • You will be asked to bring a photo ID, completed LPN Application, GED or sealed high school transcript, to the KNAT test. A diploma may be presented to test only. An official high school transcript or GED as well as two references on a school reference form, must be on file prior to an admission interview.

  • KNAT test standards for Herkimer BOCES Adult LPN placement:

    • Full-time: overall high basic 50 reading 50 math 50 ELU

    • Part-time: overall basic 50 reading 50 math 50 ELU. Remediation can be obtained through our Adult Education Department: 315-867-2085

PLEASE call to reschedule 24 hours before your scheduled test date if you cannot attend.

If you are not successful on your initial attempt, you must wait 30 days to retest. You will not be charged for a secondary retake if scheduled within one year of the billing cycle charged.

All no shows will be charged the $100 testing fee again and will only be allowed to reschedule one time

Required Application Forms

Please be aware there are physical and vaccination requirements based on the New York State Department of Health including MMR, DTAP, TB screening and COVID in order to participate in all clinical portions of your schooling. Clinical participation is a mandatory part of your schooling.
